Consultant Pharmacist - [Marlborough, MA] Hours: Mon-Fri 9am-5:30pm The
Consultant Pharmacist will be responsible for, but not limited to:
- Manage the pharmacy consulting services for Skilled Nursing Facilities in the state of operations
- Responsible for the implementation and maintenance of Partners Pharmacy policy and procedure manual at assigned facilities.
- Review patient medication records per federal/state regulations and contractual agreement
- Generate consultation reports for Nursing Facility staff including Administrator, Director of Nursing, Medical Directors, Physicians, and others.
- Perform a nursing unit inspection for each nursing station serviced by Partners as designated in facility contract and federal/state regulations.
- Develop, implement, and revise the facility emergency supply list as needed in collaboration with the Medical
- Director of Quality Assurance Committee.
- Upon request attend facility in-services.
- Ensure that intravenous medications are being used appropriate, efficacious, and safely during periodic review of medication regimen.
- Submit monthly activity report.
- Available for medication interview during JCAHO surveys
- Receive notification promptly when any potential or actual pharmacy related deficiencies are presented at the surveyor exit interview.
- Receive assisgnments that can include facilities, clinical projects, in-services and mothly regional meetings from Regional Consultant Clinical Manager.
- Maintain files on completed medication regimen reviews, nursing station inspection reports, quality assurance reports, special hospital reports, drug information files, state/federal regulation files, pharmacist to nursing in-services and other material that is required for clinical use or documentation of activities.
- Serve as a member of the center's Quality Assurance Committee and the Pharmacy Services Subcommittees.
Education: Bachelor's Degree in Pharmacy or Doctorate in Pharmacy
Professional Experience: - Registered Pharmacist license in the state of operations
- Experience in Long Term Care setting
- Travel to facilities within assigned territory
- Excellent Organizational and Communication (both written and verbal) skills
- Experience with State Survey criteria
