Talent4Health is seeking a local contract nurse RN Acute Care Case Management for a local contract nursing job in Morristown, New Jersey.

• Shift: 8:00 AM – 4:00 PM (may require weekend coverage)
• Location: New Jersey (NJ candidates preferred / local candidates strongly encouraged)

Job Overview:\nCoordinate patient care throughout acute care hospitalization.\nManage all aspects of the discharge planning and transition process.\nPerform utilization management and quality screenings.\nIn-Patient Case Management experience is required.\nRequired Skills & Experience:\nMinimum of 2 years of acute care experience strongly preferred.\nStrong knowledge of Case Management and Managed Care principles.\nExperience in In-Patient Case Management is mandatory.
