About the Role:

The Registered Nurse (RN) plays a critical role in delivering high-quality nursing care within long-term care and skilled nursing facility settings. This position focuses on providing comprehensive direct patient care, including assessment, planning, implementation, and evaluation of nursing interventions tailored to individual patient needs. The RN will serve as a patient advocate, ensuring that patients receive appropriate education about their health conditions and treatments to promote optimal outcomes. Additionally, the role involves specialized wound care management and telemetry monitoring to support patients with complex medical and surgical needs. Ultimately, the RN contributes to a multidisciplinary healthcare team to enhance patient safety, comfort, and overall well-being in a compassionate and professional manner.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice.
  • Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Basic Life Support (BLS) certification.
  • Minimum of 1 year experience in long-term care or skilled nursing facility settings.
  • Proficiency in direct patient care and medical/surgical nursing principles.

Preferred Qualifications:

  • Certification in Wound Care (e.g., Wound Care Certified - WCC).
  • Experience with telemetry monitoring and cardiac care.
  • Advanced certifications such as Advanced Cardiovascular Life Support (ACLS).
  • Bachelor of Science in Nursing (BSN) degree.
  • Experience in patient education and advocacy programs.

Responsibilities:

  • Provide direct nursing care to patients in long-term care and skilled nursing facility environments, including administering medications and treatments as prescribed.
  • Perform comprehensive patient assessments and develop individualized care plans based on clinical findings and patient needs.
  • Monitor patients using telemetry equipment and respond promptly to any changes in cardiac or vital signs.
  • Deliver patient education on disease management, medication adherence, and post-discharge care to promote health literacy and self-care.
  • Manage wound care treatments, including assessment, dressing changes, and documentation of healing progress.
  • Advocate for patients’ rights and needs by collaborating with physicians, therapists, and family members to ensure coordinated care.
  • Maintain accurate and detailed medical records in compliance with regulatory standards and facility policies.
  • Participate in quality improvement initiatives and staff education to enhance nursing practices and patient outcomes.
