Job Summary

  • The Home Health RN Case Manager provides one on one care and skilled services to patients in their own home, contributing to the individual’s overall quality of life. Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient’s overall Plan of Care. Case Management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manager their conditions and remain at home.

Job Requirements

 

Education

  • Required: Graduate of an accredited nursing program

Experience

  • Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.

Skills

  • Ability to provide reliable transportation as required by nature of position.

Licensure/Certification/Registration

  • Required: RN License active in the state of Missouri 
  • Required: BLS must be obtained within 90 days
  • Required: Active Driver's License
