The Social Work Case Manger (SWCM) utilizes professional clinical social work skills to assess patients and support systems for discharge planning, psychosocial and counseling support needs; establishes plans for effective management of the identified needs (episodic and population-based). Assessment, planning, intervention and reassessment are performed in a collaborative environment, in which the SWCM works closely with Medical Staff, Clinical Case Managers, Nursing, Allied Health disciplines, hospital business/financial services, agencies across the continuum (internal and external) and payors. In addition, the SWCM assumes a leadership role in the identification and management of patients with catastrophic needs, social determinants of health and expands collaboration with other service providers within the hospital and community exemplifying proactive, patient-oriented clinical practice that optimizes quality of care, utilization, effective cost and patient experience.
- Require a Master's Degree in Social Work (MSW) from a Council of Social Work Education (CSWE) accredited university.
- Require a minimum of one (1) year of hospital acute care or other related healthcare experience; or completion of a one-year acute care MSW internship
- Prefer active Case Management Certification (CCM, ACM-SW, A-SWCM) or within one (1) year of hire and membership in relevant CM organization.
- Prefer demonstrated ability to manage high risk situations, multiple tasks and adjust priorities according to patient needs and department/hospital needs.
- Prefer solid clinical social work assessment, critical thinking, problem-solving, interviewing, decision-making, negotiation, conflict management and organizational skills.
- Prefer strong ability to communicate effectively both verbally and in written work, and when on-call to respond to issues, patient needs.
- Prefer strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare team, payers and external agencies.

With the opening of Sarasota Memorial Hospital-Venice in fall 2021, we're able to provide south Sarasota County residents convenient access to the same level of care and 5-Star physicians, staff and systems available at our SMH-Sarasota Campus. SMH-Venice features 178 private rooms, a 28-room Emergency Care Center, 8 surgical suites, 10 birthing suites and a pandemic-ready intensive care unit. The Venice campus is currently undergoing an expansion that will double the hospital's capacity when complete in 2024.
