This role involves collaboration with patients' attending physicians to ensure seamless care delivery. The physician will guide and educate the palliative care team, ensuring adherence to palliative care standards, policies, and procedures while also enhancing the knowledge of others in the healthcare system about the benefits of palliative care. Consult on palliative care, symptom management, and supportive care needs as requested. Assist in clarifying care goals for patients and families. Facilitate access to various models of care, such as hospice and rehabilitation, based on patient needs. Support the diagnosis, treatment, and management of complex health conditions. Engage with and educate facility staff and the community on the services provided by VITAS Palliative Care. Participate in Palliative Care team meetings and take leadership roles when required. Collaboratively develop patient care plans with referring and acute care teams. Maintain accurate patient records and documentation for medical practices and reimbursement processes. Prescribe appropriate medications and treatments in coordination with referring physicians. Order and evaluate necessary diagnostics and treatments. Lead the collaboration and supervision of the palliative care team members. Assist in crafting clinical practice guidelines to support high-quality palliative care. Foster a service-oriented approach in all interactions with patients and families. Maintain confidentiality regarding all patient and organizational information. Participate in on-call rotations as necessary. Perform other duties as assigned by management. Experience: Minimum of two years in a clinical setting. Proficient understanding of interdisciplinary team dynamics. Valuable experience in patient education, care management, and symptom management for advanced illness. Strong communication skills for engagement with various stakeholders. Proficient in computer usage and familiar with software such as Outlook, Excel, and Word. Education: Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution. Certification & Licensure: Must be licensed to practice medicine in the relevant state. Possess a valid DEA license. Ongoing training in palliative medicine, diagnostic skills, and communication techniques. Board certified or eligible in Hospice and Palliative Medicine preferred.
